What is a Closed-Loop Referral System?

A closed-loop referral system is a referral process that:
- Confirms the referral was received
- Shows whether it was accepted or declined
- Tracks next steps and outcomes
- Keeps the referring partner informed (when appropriate), so care doesn’t fall through the cracks

In public health and care management, closed-loop referrals reduce delays, prevent duplication, and help ensure people actually get connected to services.

How Closed-Loop Referrals Connect to DHCS (Medi-Cal)

California’s Department of Health Care Services (DHCS) supports coordinated care models that require strong referral pathways—especially for high-need Medi-Cal members.

Closed-loop referrals support DHCS-aligned goals like:
- Whole-person care (medical + behavioral + social needs)
- Care coordination across providers and community-based organizations
- Accountability and documentation for service connection and follow-through
- Better continuity for members experiencing homelessness, instability, or complex needs

UAG’s referral process is built to support this kind of coordination—fast, trackable, and client-centered.

Who Should Use This Form?

Use this form if you are:
- A health plan, medical group, clinic, hospital, or care team member
- A county program or contracted provider
- A school, shelter, outreach team, or community-based organization
- A family member, advocate, or self-referring individual

UAG supports Medi-Cal members and community members who may be experiencing:
- Homelessness or housing instability
- High barriers to care access (transportation, documentation, phone instability, etc.)
- Behavioral health needs and/or desire for peer-led support
- Pregnancy/postpartum needs and family stabilization goals
- Employment and workforce readiness barriers

What Happens After You Submit

Here’s what you can expect from our closed-loop process:
1. Referral received (confirmation)
2. UAG reviews eligibility and urgency
3. Outreach to the member (using the contact method provided)
4. Connection to services or warm handoff to the right resource
5. Referral status updated (when appropriate)

Response times vary based on urgency, contact availability, and member consent.
